Employee of the Month

Providing love, dignity and respect to our residents requires a total team effort. Every department from nursing, dietary, transportation, maintenance and administration working together to provide care for residents. Every team has their MVP’s and our employee of the month is our chance to honor those who have given so much to the senior of York county.

Jennifer Gann - Director of Nursing

We are pleased to present our Employee of the Month for January 2017, Jennifer Gann. As Director of Nursing at Autumn House West, Jennifer has proven herself an outstanding staff leader and resident care provider. She is proactive and responsive with resident care and fluidly fills in any need, whether in Personal Care Assistance, Meds, or Hospice, as well as all the support work behind the scenes. She prides herself in giving high-quality care to residents in a home and family-like environment, and she is supportive of her nursing team as a co-worker and a friend. She also goes above and beyond her job by bringing her kids in for arts and crafts with residents on the weekends, and taking calls even when she is not on shift. Jennifer is an amazing person, and we are so glad to have her here with us!

Free Move-In Assistance

When a resident chooses to live with us at Autumn House we try to make the transition as simple and efficient as possible. One critical component of that service is our FREE MOVE IN ASSISTANCE. When a resident is ready to join us our capable staff will schedule and coordinate their move to their new home and the burden of moving becomes our opportunity to help.

Personal Care
We offer three levels of care based on your individually assessed needs, including assistance with activities of daily living, housekeeping, and medication administration. As your needs change, we can modify the level of assistance. Licensed LPNs and resident assistants are there for you 24 hours a day. Mail and newspapers are delivered to your room.
Dementia Care
Laurel Court at Autumn House West provides a supportive environment for those living with early to mid-stage dementia. You won’t find a more comprehensive secure-care residence or a more thoroughly trained staff.
Independent Living
Live independently, and in style, in our modern spacious one-bedroom apartments. At The Apartments of Autumn House East, enjoy come-and-go-as-you please living and the peace of mind and security of personal care provided by on-site staff right next door. For your convenience, two meals are served daily. (More information)
Activities
Engaging activity programs events are scheduled daily by our in-house Activities Planner. Don't miss our bus trips for shopping and outings!
Respite Care

Respite Care at Autumn House provides a short term break that can allow a caregiver to relieve stress and restore energy. The respite stay includes all base services and is charged at a daily rate. For additional information please contact the Community Liaison.

Television & Wifi
We provide expanded basic cable TV package and high-speed wireless internet access for all your entertainment needs.
Laundry
With everything from laundry and housekeeping to washers and dryers for personal use, we have your needs covered.
Transportation
We provide scheduled transportation to appointments. In addition, our wheelchair-lift-equipped bus travels to shopping and entertainment.
Dining
All plans include three chef-prepared, home-cooked meals per day.
Furnishings
All Personal Care and Memory Care rooms are furnished or you may bring your own furniture.

Laurel Court, our 13-room, secure-care residence at Autumn House West, provides a safe, supportive, homelike environment for family members living with early to mid-stage dementia. You won’t find a more comprehensive residence for peace-of-mind and dignified personal care.

We’re dedicated to helping our residents lead fulfilling and productive lives through daily engagement, sharing and activity, and to helping families cope and adjust while supporting lifelong relationships and friendships.

Laurel Court staff is professionally trained to meet the special needs of our residents. Each staff member participates in continuing education to remain abreast of changes and advances in dementia care.
